What are your favorite "accessibility red flags"?

In other words, when you first access a website, device, service, physical space, etc., what is something that immediately warns you, this won't go well for some disabled people?

I'd love to hear various perspectives, so please boost if you're willing.

Well, I'd say for physical space, furniture sorted in a way that makes it hard to understand the physical space a person is in. For apps or websites, custom overlays, or in the case of an app something important being unreadable, or a feature that's supposed to be used in a complicated way, not friendly or easy to use. Like, way too many things, focus, etc.
